Offered to patients who have been discharged, providing them with continuing health care and psychological support. This is carried out by an integrated multidisciplinary working team in order to ensure follow up of the patient’s individual cases, track progress, and avoid relapses. Importantly, this is done in cooperation and coordination with the patient’s family and other parties concerned.
